[sc34wg3] ISO 13240: ISMID

ISMID was originally requested by the US DoD for doing IETMs (Interactive
Electronic Training Manuals). I checked with Doris Bernardini (former head of
US NB, works for DoD), and she says DoD no longer uses ISMID and doesn't care
whether we maintain it or not.
