Actually, XTM is not silent on this, it just says we decode the %HH-encoded characters and don't do anything else. So implicitly it's saying that we don't transform "+" characters in any way.
I admit that if you'd asked me I'd probably have said that we do turn "+" into " ", but that's not what the spec says. And now that I look into it, neither does RFC 3986 nor XML Base or any other similar specs. So it seems that to be conformant with everyone else we shouldn't interpret the "+" in any way.

seems pretty unambiguous on this point, so I'm not sure we need to actually touch the specs in any way, but I'm open to argument on this.
